The OZEMPIC pen
There are three, and each delivers one dose size. The 2 mg/3 mL pen gives 0.25 mg or 0.5 mg, the 4 mg/3 mL pen gives 1 mg, and the 8 mg/3 mL pen gives 2 mg. Each holds four weekly doses.1
What is in the carton
The label sets this out product by product, and it is the quickest way to tell which pen you are holding: read the total strength on the box rather than the dose you were told to take.
| Total strength | Dose per injection | Doses per pen | In the carton |
|---|---|---|---|
| 2 mg / 3 mL | 0.25 mg or 0.5 mg | Four of 0.25 mg and two of 0.5 mg, or four of 0.5 mg | 1 pen, 6 needles |
| 4 mg / 3 mL | 1 mg | Four | 1 pen, 4 needles |
| 8 mg / 3 mL | 2 mg | Four | 1 pen, 4 needles |

Four weekly doses is a month, which is what makes the pen and the month line up in a price: the manufacturer’s published prices for OZEMPIC are quoted by the month. The first pen is the one that behaves differently, because it covers both of the first two steps of the schedule: four weeks at 0.25 mg and then two at 0.5 mg, or four weeks at 0.5 mg if a person is already there. That is where the sixth needle goes.1
There are also single-dose prefilled syringes, at 0.25 mg, 0.5 mg and 1 mg, supplied four to a carton. They are one dose each, so nothing in them is left over.1
A pen belongs to one person
This one is a warning in its own right rather than a handling note, and the label states it twice:
Each OZEMPIC multi-dose pen is for use by a single patient. An OZEMPIC pen must never be shared between patients, even if the needle is changed.
Changing the needle is not enough, which is the part that surprises people. It is the reason the pen carries its own numbered warning section in the label, alongside pancreatitis and the rest.1
Which pen for which step
The schedule and the pens are two halves of one thing. The first four weeks are 0.25 mg, then 0.5 mg, and both come out of the same pen. The 1 mg step needs the 4 mg/3 mL pen and the 2 mg step the 8 mg/3 mL pen, so each of those increases arrives as a new prescription rather than a new instruction. The OZEMPIC dosage chart has the steps and the waiting periods between them.1
Where the injection goes, how to rotate sites, and the pen steps in the order the Instructions for Use give them are on the page on how and where to inject. What happens if a week is missed is on the missed-dose page, which quotes all three products’ instructions.
Questions
How many doses are in an OZEMPIC pen?
Four weekly doses in each of the three pens. The 2 mg/3 mL pen can be four doses of 0.25 mg and two of 0.5 mg, or four doses of 0.5 mg; the 4 mg/3 mL pen holds four doses of 1 mg and the 8 mg/3 mL pen four doses of 2 mg.1
Can one pen give different doses?
Only the first one, and only across the two lowest steps: the 2 mg/3 mL pen delivers 0.25 mg or 0.5 mg. The 1 mg and 2 mg pens deliver one dose size each, so moving up to them means a different pen.1
Can two people share a pen if they use their own needles?
No. The label says each multi-dose pen is for use by a single patient and that a pen must never be shared between patients, even if the needle is changed.1
