The Semaglutide Report

If you missed a semaglutide dose

The approved labels do not give one answer, and which applies depends on the product you were prescribed. OZEMPIC allows five days. WEGOVY injection turns on whether the next dose is more than two days away. WEGOVY tablets say skip it.1,2

Set your week

No week set. Answers below are for a missed dose at every step, though a gap of two or more in a row is a different question.

Timing

Every instruction below is counted in days, and the count that matters is not how long ago the dose was: for WEGOVY injection it is how long until the next scheduled one. Work out that number first. It is the number the labels are written around, and it is the number a prescriber will ask you for.

What each label says, in its own words

Semaglutide is not one product with one schedule, and a missed dose is where that stops being a technicality. The two prescribing-information documents this page cites are listed at the foot of it. Both are the manufacturer’s own label as published on DailyMed, and each prints the date it was last revised. Here is what each one says about a missed dose, in its own words rather than ours.

If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once-weekly dosing schedule.

If one dose of WEGOVY injection is missed and the next scheduled dose is: more than 2 days away, administer WEGOVY injection as soon as possible. less than 2 days away, do not administer the WEGOVY injection dose. Resume dosing on the regularly scheduled day of the week.

WEGOVY- semaglutide injection, solution; WEGOVY- semaglutide tablet, section 2.4, Recommendations Regarding Missed Dose(s) — WEGOVY Injection. Updated June 18, 2026 — label Revised: 6/2026. The label prints the two conditions as a nested list under the opening line; the line breaks here are its list items and no bullet character has been added. https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=ee06186f-2aa3-4990-a760-757579d8f77b

If a dose of WEGOVY tablets is missed, skip the missed dose and take the next dose the following day.

WEGOVY- semaglutide injection, solution; WEGOVY- semaglutide tablet, section 2.4, Recommendations Regarding Missed Dose(s) — WEGOVY Tablets. Updated June 18, 2026 — label Revised: 6/2026. https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=ee06186f-2aa3-4990-a760-757579d8f77b

The three are not variations on one rule. OZEMPIC’s counts forward from the dose you missed and gives you a five-day window to take it late. WEGOVY’s injection instruction counts backward from the dose you have not taken yet, and the answer flips at two days. WEGOVY’s tablet instruction does not involve a window at all, which makes sense for something taken daily rather than weekly and makes no sense at all if you apply it to an injection.1,2

Two or more in a row is a different question

A single missed week and a run of missed weeks are separate paragraphs in the WEGOVY label, and the second one is not about the dose you missed. It is about the dose you go back to.1

If 2 or more consecutive doses of WEGOVY injection are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions [see Dosage and Administration (2.2), Warnings and Precautions (5.6), Adverse Reactions (6.1)].

WEGOVY- semaglutide injection, solution; WEGOVY- semaglutide tablet, section 2.4, Recommendations Regarding Missed Dose(s) — WEGOVY Injection. Updated June 18, 2026 — label Revised: 6/2026. https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=ee06186f-2aa3-4990-a760-757579d8f77b

That is the answer to the question people actually arrive with after a long gap, which is usually some version of “can I just pick up where I left off?”. For WEGOVY injection the label says the escalation is restarted at a lower dosage, and it gives the reason: the gastrointestinal reactions the escalation exists to keep in check. What “a lower dosage” means for you is a decision the label routes to section 2.2 and, in practice, to your prescriber. When those reactions tend to peak across the escalation, and how long they last, is on how long semaglutide side effects last.

Why the answer depends on the form as well as the brand

The WEGOVY document above is one label covering an injection and a tablet, and it gives them different missed-dose instructions in the same section. That is worth knowing before you go looking for “the semaglutide missed dose rule”: there is no such thing at the level of the molecule, and the form is part of the answer rather than a detail underneath it.

It is also a question a price page frequently does not settle. Whether a seller says it is a shot or a tablet counts how often the words printed around a published semaglutide price name the form at all, in the sellers’ own vocabulary. What the dose schedule looks like when nothing is missed is on the semaglutide dosage chart.

The escalation steps this page refers to.
StepWeeksWeekly dose
1wk 1–40.25 mg
2wk 5–80.5 mg
3wk 9–121.0 mg
4wk 13–161.7 mg
5wk 17+2.4 mg

Questions

Can I take the missed dose as soon as I remember?

Only within the window the label for your product gives, and the two labels give different windows. The OZEMPIC instruction allows a late dose within 5 days of the missed one. The WEGOVY injection instruction allows it only when the next scheduled dose is more than 2 days away. Both instructions are quoted in full above, and which applies depends on what you were prescribed.

Should I take a double dose to catch up?

Nothing quoted on this page says to. All three instructions describe taking one dose or skipping one dose, and none of them describes adding anything to a later dose. If you are unsure what to do with a gap, that is a question for your prescriber or pharmacist rather than for a reference page.

I missed several weeks. Do I restart at the beginning?

For WEGOVY injection the label says the dosage escalation is reinitiated at a lower dosage, and it points to its own section 2.2 for what that means. It does not say “at the beginning”, and this page does not either. The OZEMPIC label carries no equivalent sentence, so for that product there is nothing here to quote and the question belongs to your prescriber.

Does any of this apply to compounded semaglutide?

This page makes no claim about it. The instructions quoted here are from the prescribing information for two specific approved products, and a compounded preparation is neither of them and has no approved prescribing information for this page to quote. The instruction that governs a compounded prescription is the one that came with it.

If the question is who pays for it

Coverage, prior authorization and compounded prescriptions are separate pages

That is a different subject with its own route, and it is a pathway rather than a yes or a no — what decides whether a plan covers semaglutide sets out what a plan is actually deciding and how to find out for your own. Prior authorization covers what happens when the plan asks for something first, and a compounded prescription is a separate question again. What it costs before insurance enters into it at all — by dose, not as one headline number — is on what semaglutide costs by dose.

When to call your prescriber

Call if you do not know which of the three instructions above is yours, if the gap has run to two or more doses, or if you are being asked to decide something the label routes to a clinician. Your prescriber and your pharmacist can both see which product you were given and how long the gap has been, and neither of those is something a reference page knows about you.

If you think you are having a medical emergency, that is not a question for a page at all. Call your local emergency number.

A missed week is a question about where you are in the ladder, which is the same thing the dosage chart sets out step by step. If the dose you return to is a step you have already had trouble with, nausea on semaglutide covers where in a step it usually turns up, and what to eat covers what people adjust after an increase.
