The Semaglutide Report

WEGOVY pen or pill, by price

At the maker’s published self-pay prices the tablets come out cheaper: about $3,338 over a first year against $3,888 for the pen. The pill costs you a daily routine the injection does not.2,1

The two price ladders, side by side

Both sets of figures are Novo Nordisk’s own, from its page for this drug, read 11 September 2026. A month means one box of four pens, or one bottle of 30 tablets. Each form also has its ladder in full on its own page: what the WEGOVY pen costs and what the tablets cost.2

Published self-pay prices for each form, by stage of the schedule.
StagePenTablets
First step$199 a month, 0.25 mg and 0.5 mg, for two fills$149 a month, 1.5 mg, for 30 days
Second step$349 a month from 1 mg$199 a month, 4 mg, for 30 days
Third step$349 a month$299 a month, 9 mg, for 30 days
Maintenance$349 a month at 2.4 mg, $399 for the 7.2 mg pen$299 a month at 25 mg

Wegovy.com, “Save on Wegovy”, read 11 September 2026; the page prints no revision date and states that pricing is to be updated after 31 December 2026. Doses from the WEGOVY label, Tables 1 and 2 (revised 6/2026).

Add up a first year at the label’s minimum pace and the difference is easier to see than it is month by month. Tablets: one month at $149, one at $199, one at $299 and nine at $299, which is $3,338. Pens: two months at $199 and ten at $349, which is $3,888. The gap is $550.2,1

What the cheaper form asks of you

The tablets are a daily medicine with a rule attached, and the rule is the reason people abandon them:

Take one WEGOVY tablet orally once daily on an empty stomach in the morning with water (up to 4 ounces).

WEGOVY prescribing information, section 2.3. 1

After that comes a wait of at least 30 minutes before eating, drinking anything else, or taking other tablets. Thirty mornings a month against fifty-two injections a year is a real trade, and it is not the one a price table shows you.1

The two ladders also run on different clocks. The pen moves every four weeks; the tablets every 30 days, ending at 25 mg rather than 2.4 mg. The WEGOVY dose schedule has both in full.

Sometimes the choice is not open

The forms are not approved for the same things. Both carry cardiovascular risk reduction and weight reduction in adults, but the tablets do not carry the liver indication, and the weight use for patients aged 12 and older is the injection’s. The page on the shot and the tablet sets out where the two labels part company.1

There is also a route back. If a patient does not tolerate the 25 mg maintenance dosage, the label says to consider switching to WEGOVY injection at 1.7 mg once weekly, which at these prices is a move from $299 a month to $349.1

If you are not paying the whole thing

Everything above is the self-pay case, and whether a plan covers WEGOVY at all is decided before any of these numbers apply. With commercial insurance the manufacturer’s savings offer applies to both forms and is capped at $100 of savings a month, and eligible Medicare patients pay $50 a month through the GLP-1 Bridge program. Which program you qualify for sorts those out, and the discount-card comparison explains why a coupon does not beat these numbers.

Questions

Is the WEGOVY pill cheaper than the injection?

At the manufacturer’s published self-pay prices, yes. At maintenance it is $299 a month against $349, and over a first year on the label’s minimum schedule the totals are $3,338 for tablets and $3,888 for pens.

Why is the first month so much cheaper?

Because both ladders start low and the prices follow the dose. The pen’s $199 is an introductory price for a new patient’s first two fills at 0.25 mg and 0.5 mg; the tablets’ $149 is the price of the 1.5 mg strength, which is the first 30 days.

Can I switch from the tablets to the pen?

The label names that move for one situation: if a patient does not tolerate the 25 mg maintenance dosage, it says to consider switching to WEGOVY injection 1.7 mg once weekly. Any switch is a prescriber’s decision about dose as well as form.
